A picture must be submitted for acknowledgment on website. The individual who nominated the winner will be notified by email. It is up to them to notify the winner and have their parent or legal guardian submit a photo of the winner and photo release form to bighedsclub@volarellc.com (Golden Strawberry Award Winner must be in email subject). Nomination deadline is the 15th of each month and photo and photo release form must be received by the 28th of each month. Individual who nominated the winner will be notified by the 20th of each month. Please click here to download the photo release form.
